So, I personally headcanon that the night elves create their tattoos the same way as Filipino kalinga tattoos. Basically, it's two sticks: one with a needle or thorn on the end and one that's blunt. Dip the thorn or needle in wet charcoal (colored charcoal in the night elves' case) and tap the needle into the skin repeatedly with the other stick. Ouch.
